SPELLING AND STYLE: Consult either Webster's New Collegiate Dictionary or, for technical terms, Dorland's Illustrated Medical Dictionary. For proper manuscript style, the AMA Style Manual is preferred. Please use
American spellings.

TABLES: Cite all tables in the manuscript in numerical order. Only actual tabular material should be submitted
in table form. Classifications and simple lists should be incorporated into the text itself.
FIGURES: Illustrations should
appear at the end of the manuscript and not in the text. Cite each figure in the manuscript in numerical order.
